Premium Assistance Program: 
 

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires that applicants are eligible if:

  • They have had 18 or more months of creditable coverage, the last of which must be group coverage

  • Have received notice of termination of comprehensive medical/surgical group coverage (including COBRA continuation) that terminates within 63 days prior to the initial application date.

Proof of such existing coverage must be included with the application for NMMIP coverage.

There are no family rates-, each family member who is enrolled will be charged the rate applicable to his/her age and sex.

The following persons may be covered under a NMMIP family policy if they each complete an application and satisfy all eligibility requirements:

  • Your spouse

  • Unmarried dependent children, under age 19

  • Unmarried dependent children from age 19 to 25 maybe eligible if they are  full-time students at an accredited educational institution.

The age limit does not apply when the eligible, unmarried, dependent child is incapable of self-support employment by reason of mental retardation or physical disability and is primarily dependent upon the policyholder for support and maintenance.

IMPORTANT:  Coverage is not available to persons who have or are eligible for Medicare, Medicaid or a substantially equivalent coverage under health insurance or other health plan. Other coverage may be considered to be substantially equivalent to NMMIP coverage even if the NMMIP coverage provides for some procedures or conditions that the other coverage does not or provides a higher reimbursement level for some benefits.
